Exempt Withholding Forms Expire on February 15, 2025:
- Any federal or state form claiming exempt from withholding expires on February 15, 2025. Per federal law, a 2025 W-4 form as well as your corresponding state form must be received by February 15, 2025. If you do not complete the forms, the Consumer Direct Care Network (CDCN) Payroll Department will begin withholding taxes at the single or married filing separately status. Please fill out and return your 2025 W-4 and state form as soon as possible to prevent delays.
Effective January 1, 2025:
- Attendants must document daily tasks performed during each shift in the Fiscal/Employer Agent (F/EA) system regardless of live in status. Payment will only be made for services that are documented in the plan of care. Unauthorized tasks performed by the Attendant may be subject to payback.
- Attendants must submit tasks performed and shifts within 30 calendar days from the date of service. If shifts are not submitted within 30 calendar days, the shifts may be denied unless the shifts meet one of the four (4) exceptions. Review this FAQ document for a list of exceptions.
2024 W-2s are coming soon! We will mail W-2s on or before January 31, 2025. Once they are ready, you can also access your W-2 online through ADP.
There are two ways to access ADP.
- Visit myADP.com directly.
- Log into the DirectMyCare web portal and click the link on the My Dashboard screen.
The first time you visit ADP, you must set up an account. Set up your ADP account today!
Note: If you went paperless for your W-2, you will not receive a paper copy in the mail.
