COVID-19: Temporary Attendant Forms

We are glad to share that parents and spouses have been approved to be temporary Attendants in response to the COVID-19 pandemic. Prospective Attendants must fill out an electronic enrollment packet. You can find the electronic enrollment packet and instructions below.

  • All temporary Attendants must enter time on the web portal.
  • No paper timesheets will be accepted.
  • The current Employer of Record (EOR) cannot be a paid Attendant. If there needs to be an EOR change, the Service Facilitator will need to submit a Fiscal Agent Request Form (FARF) and the EOR and Attendant will need to submit the required enrollment forms.
