How should I report a change in the Participant’s physical address?

Any changes in the Consumer’s physical address must be reported to the local Department of Social Services (DSS) office. Please contact your eligibility worker to report address changes. CDCN will receive the new address automatically from DSS.

You can use DSS’ website at www.dss.virginia.gov/localagency/index.cgi (http://www.dss.virginia.gov/localagency/index.cgi) to find the contact information for the location most applicable to you.
