Starting July 13, 2023, CDVA and Optima Health are partnering to provide Fiscal Employer Agent (FEA) services for you and your attendant. This letter will explain how you and your attendant will enroll with us.*

Enrollment with CDVA

Starting May 30, 2023, the Employer (you or your designated representative) and attendant will need to complete these steps:

  1. Send CDVA your email address to make sure we have the most up-to-date information. You can do this by visiting www.ConsumerDirectVA.com/Email. Complete the form on the webpage and click the “Submit” button. If you want to keep the email address you had with Public Partnerships, LLC (PPL), you do not need to complete this step.
  2. You will receive an email with an electronic packet to enroll between May 30, 2023, and June 2, 2023. The email will come from DocuSign.

Make sure your attendant completes the same steps, so they can also be enrolled.

Once enrolled, attendants will be able to record their hours and the employer can approve them using the mobile app, online portal, or automated phone system. Instructions will be included in your enrollment packet. Please watch for emails from both Consumer Direct Care Network Virginia and DocuSign. If you do not receive an email from DocuSign by June 5, 2023, please contact CDVA at 1-888-444-2419 (TTY: 711), weekdays from 8:00 a.m. to 6:00 p.m. and Saturdays from 9:00 a.m. to 1:00 p.m.
